29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: ESTABLISHMENT: The employer did not ensure that authorized employees who were performing maintenance and/or repair of machines or equipment, such as (but not limited to) the Station 100 P3 Testing Equipment, Number 258957 were provided with locks, tags, or other equivalent devices to isolate and secure energy sources prior to performing the tasks.
