29 CFR 1910.333(c)(3)(ii): When a qualified person was working in the vicinity of overhead lines, whether in an elevated position or on the ground, the person approached and/or took a conductive object without an approved insulating handle closer to exposed energized parts than shown in Table S-5: (a) Modern Pump & Equipment Inc. Houtzdale Municipal Authority Pump house #4 - An employee used a crane hook to pull the wire rope closer to a spool of wire which was on the ground. This brought the wire rope closer than the 10 foot minimum approach distance from a 19.9 kV overhead power line, creating an electrical arc that shocked the employee, on or about January 2, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
