Cited "Serious" OSH Act of 1970 Section 5(a)(1) was amended to reclassify to Other-Than-Serious and amend the cited standard to: 29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished: On or about August 28, 2018 employees attempted to remove a Christmas tree from a closed gas well when there was a release of well pressure. The employer did not ensure that all pressure had been bled off prior to removal of Christmas tree.
