OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards of being struck-by overhead concrete beam from the ceiling garage which were causing or likely to cause death or serious physical harm to employees: a) The employer did not protect employees from struck by overhead concrete beams from the ceiling garage. This violation occurred on or about December 1, 2017, and at times prior thereto; at the workplace located at MCD parking garage at 7777 Forest Lane, Dallas, TX, where employees, who were driving/operating aerial platform, were not maintaining a safe distance from overhead obstacles, exposing employees to the struck-by hazards.
