Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion: On or about October 15, 2017, employees working on an oil/gas production platform located in Lake Ponchartrain LA, were engaged in a well servicing pipe cleaning/clearing operation utilizing a proprietary chemical process to remove paraffin wax. The employer did not manage the change of the process used for pipe clearing, which includes but is not limited to evaluating the contractor and the contractors assessment of the process, and analyzing and evaluating the known and recognized hazards of the process and the potential for fire and explosion prior to the commencement of the operation.
