29 CFR 1910.28(b)(9)(i)(A): Each fixed ladder installed before November 19, 2018 was not equipped with a personal fall arrest system, ladder safety system, cage, or well: (a) O&J Coatings, Inc. at 300 14th Street South East in Watertown, SD 57201: On and prior to September 2, 2017, the exposing employer did not ensure employees utilized personal fall arrest systems when descending the approximately 46.5 foot upper fixed ladder of the Larson water tower. This condition exposed employees to a fall hazards between 24 to 46.5 feet.
