Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) The hazard of being struck and crushed by the Kelly and swivel when the Bill Hook opened and released the Kelly and swivel on or about February 7, 1993. The employees were in the process of jarring pipe on a horizontal well. One feasible and acceptable abatement method, among others, to correct these hazard(s) would be to enforce and establish the following procedures: (1) Have Bill Hook and Block latch assembly repaired; (2) Develop written procedures on maintenance of Bill Hook latch following manufacturing recommendations. (3) Develop an inspection program by the driller at every shift change.
