29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about May 3, 2017, and times prior thereto, employer exposed employees to hazardous energy when they did not utilize a specific procedure for the XV484 valve to ensure the employees are properly locking out and tagging out all energy sources when performing maintenance or servicing in the Coker unit.
