29 CFR 1910.28(b)(3)(i): Holes. The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: a) Water treatment area, on or about March 29, 2017 and at times prior to, employees working near an uncovered and unprotected manhole (24 inches in diameter) was exposed to tripping and stepping into the opening that was about 10 feet deep, exposing employees to falls, caught-in and struck-by hazards.
