29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) The employer failed to adequately guard the Eubanks Tube Cutter point of operation to prevent employees from having any part of their body in the danger zone during cutting operation. This was most recently demonstrated March 22, 2017, when an employee reached into the out-feed side of the tube cutter to clear pieces of tub and received a finger tip amputation. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
