29 CFR 1910.334(a)(2)(ii): If there was a defect or evidence of damage that could have exposed an employee to injury, the defective or damaged item was not removed from service, and an employee used it before repairs and tests necessary to render the equipment safe were made: On or about February 10, 2016, at the above addressed jobsite, employees were exposed to an electrical hazard when using an extension cord whose outer sheath insulation was damaged, exposing the internal conductors.
