29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about February 5, 2015, and at certain times before, employees working in the sanitation department were exposed to serious injuries, including amputations, while performing sanitation work on industrial meat processing machinery, including but not necessarily limited to the Cozzini meat blender described by the company as "Blender 1, Tunnel 1", due to inadequacies in machine-specific lockout/tagout procedures. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
