Investigation Summary
At 3:30 p.m. on June 21, 2021, Employee #1 was performing initial plastic bag setup on a Triangle bagging machine (Model XM15C/A61254F2, Serial #124533/124532). He noticed the plastic bag was jamming on the seal bar and opened the front doors of the machine to remove built-up plastic. During the process, the seal bar closed and made contact with his right hand, resulting in a laceration to the middle fingertip. The employee was treated without hospitalization.
