Investigation Summary
At 11:15 a.m. on April 20, 2020, while attempting to clear a plugged flour conveying line, a fabricated plug was clamped to the end of the pipe. Attached to the plug was a male air hose coupling. When the maintenance technician attached a compressed air hose to the male, the coupling exploded and struck the employee on the arm. The employee sustained injuries to multiple body parts, including a broken hand. The employee was hospitalized.
