Abstract: At 10:40 p.m. on October 8, 2017, Employee #1, a furnace operator, noticed an alarm that indicated that the material was not flowing. Employee #1 told a coworker that he was going outside to check on the problem. As Employee #1 went outside with a ladder to look into the problem, he inadvertently clipped his finger tip on a rotary valve that was in operation and not locked out, resulting in its amputation.
