Abstract: At 8:30 a.m. on April 4, 2017, an employee was at a propane filling station. He was refueling the propane tank of the forklift he had been driving. The employee completed re-filling his eight gallon propane tank and when beginning to drive away from filling station, he heard a hissing noise and stopped while leaving forklift running in idle. The employee got off his forklift and noticed propane was spewing out of one of the 4-100 gallon cylinders hoses that is used to re-fill propane tanks. A kiln located next to the propane tanks then ignited the gas and the employee was burned. Manuel Rivera Perez (Injured Employee) sustained 2nd and 3rd degree burns to 65% of his body, including head, neck, face, ears, back chest, arms and hands.
