Abstract: At 10:45 p.m. on June 19, 2017, an employee was operating a pallet jack when he inadvertently crashed into a material storage rack. The employee was crushed between the jack and storage rack and sustained a punctured lung, lacerated liver, nerve damage to his right arm, and broken ribs on his right side.
