Abstract: At approximately 10:30 a.m. on June 30, 2017, an employee was beginning to repair a leaking pneumatic fitting of an abrasive blasting machine. He shut off blasting machine and disconnected the media hose before walking to another building to get a wrench to remove the worn fitting. While the employee was away from the machine, a coworker turned the machine back on and walked back to his work area. When the employee returned and was lying in front of the fitting, the coworker triggered the valve to open. The employee was injured when the machine was not locked out and it re-energized. The employee was hospitalized and treated for his injuries.
