Abstract: At 1:15 p.m. on January 16, 2017, an employee was instructed to gather and then grind scrap sheets of nylon that had been processed via slitting and punching. The employee began feeding this scrap material into the hopper of a Rapid Model 150-35 Granulator. These scrap sheets had a degree of curling, which were of varying lengths & widths, and round plastic gaskets that were cut out of some of these sheets leaving holes in these sheets. The sheets in a pile were bird-caging and the employee's left ring finger got caught in a hole of one of these sheets. That particular sheet was pulled downward into the hopper by the action of the grinder. The employee's finger was amputated by the sheet as the downward force pulled the sheet along against an inside surface of the hopper. The employee also fracture the index finger.
