Abstract: At 12:30 p.m. on October 31, 2015, a worker was using a plasma cutter to remove the lid of a 55 gallon barrel. The barrel previously contained toluene. As the worker was cutting the barrels lid, an explosion occurred that caused the lid to be ejected, striking the worker in the head. The worker suffered severe blunt force trauma injuries to his head and eyes, resulting in his death.
