Abstract: At 11:27 a.m. on February 8, 2015, an employee was placing cases of ribs to be cut on the Hobart vertical meat saw table, after removing the ribs the employee reached passed the exposed saw blade to discard the empty boxes and pick up box cutter. The employee had not turned off the Hobart vertical meat saw after meat cutting was completed and had not lowered the blade guide guard. The employee reached passed the moving exposed saw blade and the right sleeve of the white butchers coat worn by the employee got caught in the vertical saw blade that travels in a downward direction pulling the employees right forearm into the saw blade. The employee's lower third of the right forearm and right hand were amputated. They attempted to reattach the lower right forearm. However, the surgery was not successful and resulted in the loss of the entire right forearm.
