Abstract: At 3:15 p.m. on August 22, 2014, Employee #1 and Employee #2 had just returned from their break. Employee #2 had been sitting in the truck with the air conditioner on and appeared to be asleep. Employee #2 opened the door of the truck and vomited, but stated that he was okay when asked. Employee #1 and Employee #2 got in an aerial lift and went up to work on the bridge. Both employees wore personal fall arrest with lanyards attached to anchor points on lift. Employee #1 stepped under the gate of the aerial lift platform onto a wooden form on the north side of the bridge. Employee #2 leaned over and fell to the ground below. He sustained multiple blunt force injuries and was killed.
