Abstract: At 9:45 a.m. on July 28, 2014, two employees were working as contract employees. With some coworkers, they were at a facility, preparing to cut piping on a "stickwater" storage tank. Stickwater is a slurry of water and fish matter. The contract workers were making a weld on piping attached to the lower part of the tank. This was hot work. They were preparing to cut piping on the top of the storage tank, when the top blew off. Employee #1 was thrown to the top of another tank. Employee #2 was thrown to the roof of a structure. Employee #1 who landed on the tank sustained massive head injury, multiple fractures, and he was killed. Employee #2 sustained multiple fractures and lacerations and he was hospitalized. It was later found that the air in the stickwater storage tank contained methane and hydrogen sulfide.
