Abstract: On September 20, 2012, Employee #1 was delivering a window to an offsite location. The employee was carrying the window out of his truck trailer, when a gust of air from ventilation equipment at the site came out of the loading dock and caused a loading strap that hung inside the wall of the trailer to catch Employee #1's foot. The employee tripped and dropped the 6-ft by 4-ft, 100 to 150 lb. window. The window frame was about 5 in. thick. A portion of the window frame landed on the employee's right foot, causing a fractured ankle. The employee was transported by the City of Ontario, Fire Department to an area hospital, where he was hospitalized for surgery for more than 24 hours and released.
