Abstract: At approximately 1:30 a.m. on April 19, 2006, Employee #1 was adjusting the offset plate of a Hanky HS-420P offset printer, when her left hand became caught between the moving offset plate and the offset roller of the printer. The printer was not deenergized, disengaged or mechanically blocked or locked while she was conducting the adjustment. She suffered an amputation and was transported to Holy Cross Medical Center and then to UCLA Medical Center, where she was hospitalized with her injury.
