Abstract: At approximately 5:00 p.m. on June 25, 2012, an employee was using an oxygen/propane cutting torch to remove the top lid from a 55-gallon steel drum. The drum previously contained methanol. The flammable vapors of methanol in the drum ignited causing the drum to explode. The employee was struck in the head by a part of the exploding drum lid. The employee was killed.
