Abstract: On June 17, 2009 Employee #1 was directing the unloading of a junk vehicle from a truck. As the vehicle was lifted from the truck with an industrial forklift, a small safe fell from the vehicle. The safe landed on Employee #1's toes, crushing them. He was hospitalized at Watsonville Community Hospital for amputation of his toes.
