Abstract: On January 8, 2010, Employee #1 was vacuuming water out of a 3 to 4 ft deep excavation, during a horizontal drilling operation. The drill rod was routed between two electrical conduits and partially severed one of three phases of one of the cables. Employee #1 was electrocuted.
