Abstract: On October 22, 2009, Employee #1 was operating a magnetron roll compression machine. The foot pedal was tied down with a bag tie, so that the machine would run continuously. When the supervisor discovered it, Employee #1 grabbed a hook blade knife and attempted to quickly remove the bag tie. In doing so, the hook portion of the blade broke off and caused Employee #1 to strike his lower left arm and wrist area. Employee #1's arm was severely lacerated. His employer took him to the hospital emergency room for medical treatment.
