Abstract: At approximately 1:00 p.m. on October 7, 2002, Employee #1 of Cal Lift Incorporated was climbing up the side of a Mitsubishi electric reach truck (Model Number ESR36, Serial Number 1ESR361108). He was trying to get to the top of the overhead protection guard to repair the mast limiting switch. He slipped off as he was climbing and fell 7.5 feet to the concrete floor. He sustained fractures to his right ankle and was transported to San Dimas Community Hospital were he was hospitalized.
