Abstract: On September 20, 2010, a drill crew was running casing, and they encountered a bridge. The crew decided to use mud to wash out the bridge. They connected a 2-inch mud hose and pumped mud down to the bridge. As they neared the bridge, pressure increased and caused the hose to kink. The driller instructed Employee #1 to pull back on the hose. While Employee #1 was pulling back on the hose, the crew again tried to clear the bridge with the mud. When they hit the bridge, the pressure spiked to over 1,000 PSI and caused the hose to kink and rupture. The sudden release of uncontrolled pressure caused the hose to whip violently and throw Employee #1 into the air. He struck the rig derrick and landed at the foot of the rig floor, approximately 25 feet away. Employee #1 was killed.
