Abstract: At approximately 4:05 a.m. on August 21, 2013, Employee #1, a 44-year-old male with Quaker Oats Company, and a coworker had just completed installing a motor for the crunch berry conveyor. The coworker unlocked the electrical disconnect, and Employee #1 climbed a rolling stairway to observe if the motor functioning properly, before putting the metal guards back on the machine. While inspecting the motor, Employee #1 lost his balance and reached out to catch his balance. He then put his hand on the conveyor, which resulted in an amputation, when his finger was caught in between a belt and pulley. The coworker directed Employee #1 to head up one floor to the maintenance shop, to get away from traffic on the production floor. On his way, the coworker told another worker what had happened and directed that worker to the maintenance shop to help Employee #1, while the coworker locked out the crunch berry conveyor and moved the rolling stairways. At that time, Employee #1 had his injured finger wrapped in towels and preceded down to the security office with a coworker. A coworker looked at Employee #1's finger and instructed him to keep pressure on his finger. Then, a coworker was called to take Employee #1 to St. Luke's Hospital in Cedar Rapids.
