Abstract: At approximately 10:38 p.m. on October 26, 1991, Employee #1 was shoveling excess scrap from around the deck of an electric arc furnace in preparation for the next batch of scrap iron to be melted. As the furnace was being tapped, water infiltrated underneath the molten steel and hydrated it, resulting in an explosion. Employee #1 was struck by hot slag and molten metal, and sustained burns that required hospitalization. It is believed that the water came from worn cooling water panels within the furnace itself. Employee #1's duties did not include operation of the electric arc furnace.
