Abstract: At 2:00 p.m. on April 6, 2022, two employees were performing well servicing on a salt water disposal well. The mobile service rig turned over, fatality injuring one employee, a floorhand who was working on the ground, and seriously injuring the other employee, a derrickhand who was working from the tubing boards and rode the rig down to the ground. The mobile service rig was not secured to permanent anchors with rated capacity. The derrickhand sustained multiple fractures and was hospitalized for treatment.
