Abstract: At approximately 7:30 a.m. on Sunday, April 22, 1990, the company president assembled a four-man crew to begin constructing a new roadway within the boundaries of the shipyard. After some initial preparation, the president began operating a truck mounted dragline while three of the crew performed other tasks. Employee #1 remained by the dragline. At approximately 10:30 a.m., Employee #1 was caught between the rotating superstructure and chassis of the dragline. No one witnessed the accident. Apparently, he was attempting to keep some hydraulic lines from fouling around the turntable, but was struck by the superstructure and crushed between it and the chassis. He died.
