Abstract: Employee #1 was working in a sitting position with his feet dangling over the open side of a two-tiered scaffold as he washed out an aircraft flap well with a soapy water solution. The scaffold was open-sided on its front because it was designed to face flush with an aircraft fuselage. The scaffold's upper elevation was 11 1/2 feet above ground while the lower level was 7 1/2 feet above ground. The second elevated level provided access to the underside of the wing of the C-130 aircraft. When Employee #1 finished his task, he attempted to slide from the upper to the lower work platform. His feet slipped out from under him as he made contact with the wet surface. He fell to the concrete, sustaining fatal multiple traumatic injuries including a fractured skull.
