Abstract: At approximately 11:00 a.m. on April 29, 2021, an employee was working as a washer for a carwash. In addition to washing vehicles, the employee also drives the vehicles up to the conveyor track system for the automatic car wash. The employee had worked for the car wash since December of 2019. The employee had been preparing a car to go on the track system when he got caught in the conveyor system. There were no direct witnesses to the incident. During the course of the investigation and interviews with employees, the Agency determined that the employee slipped and caught his foot in the three inch opening in the track system and was trapped. The local fire department responded and disassembled the car track system to release the employee's foot. The employee was admitted to the hospital to treat crush injuries to his left ankle. On April 30, 2021, the employee was discharged from the hospital.
