Abstract: At 12:08 p.m. on February 17, 2021, Employee #1 and Employee #2, temporary employees at a mattresses and bedding manufacturing plant, were working together to load scrap foam into a vertical CRAM-A-LOT baler for compaction. As the employees were pushing foam into the baler with their hands while the platen was in the up position, the platen, weighing approximately 550 lbs and operating at 2,500 psi, came loose and struck both employees' arms. Employee #1 sustained two broken arms, while Employee #2 sustained bruises on his arms. The accident occurred due to the retaining nut not being fully tightened on the piston end of the shaft, causing it to disengage from the hydraulic cylinder. Emergency services transported Employee #1 to the hospital for admission and treatment. Both employers were in compliance with reporting requirements, with the primary employer reporting the incident at 5:12 p.m. on February 17, 2021, and the secondary employer reporting it at 12:27 p.m. on February 18, 2021.
