Abstract: At 8:50 a.m. on July 16, 2021, Employee #1 was operating a rigid cell brace punch machine without the guard in place. The employee cycled the foot-activated press while their finger was in the point of operation, resulting in an open displaced fracture of the distal phalanx of the left index finger. The fingertip was later amputated to the middle knuckle during surgery. The employee was hospitalized.
