Abstract: At 2:45 p.m. on July 20, 2021, Employee #1 was disconnecting a hose on top of a tanker and closing the tanker using a cage with an attached gangway stair lowered from an overhead catwalk. While she was on top of the truck, the truck driver began moving the truck backward. With the gangway stair and safety cage still in place, her legs became caught between the cage and the trailer. Employee #1 sustained two broken bones in her right leg, a fractured left leg, and a small fracture to her spine. She was hospitalized for treatment.
