Abstract: At 2:15 p.m. on April 22, 2021, aluminum sheet metal, which was being cut with a table saw by another a coworker, flew off the saw after the cut and hit Employee #1, who was sitting at a desk about 10 feet away. Employee #1, who was not wearing personal protective equipment, lost consciousness for about 15 seconds. The piece of metal caused permanent right eye blindness, multiple facial fractures, lacerations, and other injuries, including post-traumatic stress disorder. Employee #1 was hospitalized from April 22 to April 25, 2021, and underwent surgery for the lacerations.
