Abstract: At 12:01 p.m. on Thursday, March 18, 2021, an employee was working for a broadwoven fabric finishing mill. He worked in machinery maintenance. He was removing the shaft from the center of a hollowed-out metal cylinder or roller that was part of material handling machinery. The shaft supported bearings on which the cylinder or roller rotated. Workers at the facility had developed a technique for this job. They removed the shaft from the cylinder by striking it against a hard surface. Specifically, the procedure involved ramming the metal roller into a forklift counterweight, thus dislodging the roller shaft from the cylinder. The narrative indicated that the employee was holding a hand tool. In this incident, the employee's left thumb got caught between the handle on a piston and the roller. It was crushed and partially severed. The employee was hospitalized. The thumb was amputated, apparently during a surgical procedure.
