Abstract: At approximately 4:30 p.m. on April 6, 2021, an employee was preparing to paint a damaged section of ductwork. The employee used a damaged extension ladder with worn smooth feet to access the work area. Due to the slippery flooring, the ladder slipped from under the employee, causing a fall. The employee struck his head on a prepping table and sustained a head injury. The employee was hospitalized for treatment.
