Abstract: At 9:45 a.m. on April 5, 2021, employee #1 began showing symptoms of carbon monoxide exposure, and 911 was called. The employee was transported to the hospital for treatment. At 11:00 a.m., a group of employees began experiencing headaches and vomiting, prompting another 911 call. At 11:15 a.m., the entire facility was evacuated, and a gas leak was suspected. A total of 18 employees were affected by carbon monoxide exposure. Twelve employees were transported by ambulance to McMinnville and Newberg hospitals, where they were treated and released. Three others were sent to Emmanuel Hospital in Portland for treatment in the hyperbaric chamber-two by ambulance and one by air. The two employees arriving by ambulance were released after treatment. One employee stayed overnight for additional hyperbaric treatment on April 6, 2021. Both boilers were shut down, and the plant was evacuated at 11:15 a.m. The Fire Department monitored CO levels and cleared the structure, with all CO levels at 0 ppm, except in one room where it was 10 ppm. Affected employees worked in the kitchen, retail floor, and shipping offices.
