Abstract: At 10:40 p.m. on December 13, 2020, Employee #1, a Thermoformer machine/line operator, was adjusting a machine-mounted fan that was positioned less than seven feet from the walking surface. During the adjustment, Employee #1's left middle finger came into contact with the moving fan blade, resulting in an amputation of the first digit. Although the mounted fan was equipped with a guard (shroud), the openings in the shroud were large enough for the employee's fingers to pass through and make contact with the moving blades. The employee was hospitalized. The incident occurred indoors, and weather conditions were not a factor.
