Abstract: At 3:00 p.m. on February 24, 2021, Employee #1, a dishwasher performing general cleaning duties at a closed restaurant, was cleaning grease from an exhaust fan located on the building's exterior. The employee used a personal 12-foot Little Giant Kombo ladder to access the fan. While cleaning the right side of the fan, Employee #1's legs went through the ladder's rungs, causing him to fall. The fall resulted in fractures to both of his legs. The employee was assisted by another worker, and emergency services were called. Employee #1 was transported to the hospital for admission and treatment.
