Abstract: At 7:00 a.m. on December 17, 2020, Employee #1, a 56-year-old fleet mechanic at Pepsi Beverages Company (NAICS 424490), was assisting a forklift operator in removing an electric pallet jack (EPJ) from a semi-trailer in the facility's truck holding garage. While working from inside the semi-trailer, Employee #1 stepped onto the tines of the EPJ to counterbalance the battery's weight. As the forklift raised the EPJ, it became unstable and began to tip. To avoid being struck or crushed, Employee #1 jumped off the tines, landing approximately four feet below on the concrete floor, resulting in a fractured left tibia. The employee was transported to a local hospital, where he was admitted for surgery.
