Abstract: At 5:21 a.m. on November 16, 2020, Employee #1 was attempting to fix the proximity sensor on a VMI elevator 150 bowl lift. Employee #1 deenergized the bowl lift, fixed the sensor, and reenergized the bowl lift. Employee #1 then noticed that he had left pliers under the machine. While attempting to retrieve the pliers, he inadvertently activated the sensor, causing the VMI elevator 150 bowl lift to clamp onto his right forearm. Employee #1 suffered a puncture wound to his right forearm and was hospitalized.
