Abstract: At 1:30 p.m. on November 4, 2024, Employee #1, a supervisor, was climbing into the back of a pickup truck from a Tommy lift gate to collect equipment. Employee #1 stepped onto the lift gate and then onto the truck bed, but while upset with other employees for improperly storing equipment, his foot became caught in the cross member of the gate, causing him to lose balance and fall backwards. During the fall, his foot remained caught, and he struck his head on the ground. Employee #1 lost consciousness for approximately 3 minutes before emergency medical services arrived. The employee was hospitalized and treated for a concussion.
