Abstract: At 8:15 a.m. on December 13, 2020, a maintenance worker was checking the function of an air actuated valve after it was placed in position. The incident occurred after the lockout/tagout procedure was completed, during which the worker examined the valve's operation. According to the manufacturer's recommendations, the valve should be operated several times to ensure smooth functionality while keeping hands away from ports when compressed air is supplied. The worker's left middle finger was caught in the valve mechanism, resulting in amputation. The employee was treated without hospitalization.
